Individuals usually make decisions regarding their healthcare treatment after their physician recommends a course of treatment and provides information about the treatment. Through documents, known as Advance Directives, individuals can express their treatment preferences before they actually need such care, ensuring that their wishes will be carried out and  their families will not be faced with making these difficult decisions.

Laboratory 
Our facilites offer laboratory services that are accredited by the College of American Pathologists. Lab services are available 7 days a week.

Phoenix Health Plan/Community Connection
Phoenix Health Plan/Community Connection recognizes the cultural diversity of our members and offers bilingual physicians and translation services. This service is provided to help members understand their health plan benefits and medical services. Telephone translation services are available to members in 140 languages. The health plan also provides Sign Language interpretation. Members or providers can use this service by contacting the health plan at the numbers given below, 24 hours a day/seven days a week.
