Patients who would like to obtain copies of their medical records can download the consent form below. Prior to receiving your medical record, please print, complete, sign and mail or fax the form to the appropriate hospital.

When you come to pickup your medical records, please bring a photo ID. If an individual other than the patient is picking up the records, then that individual will need an original signed authorization letter from the patient and a photo ID. There may be a charge for creating a copy of your records.
