Most office visits are an average of 10 minutes but at Paradise Valley Hospital Outpatient Diabetes Clinic, our nurse practitioner spends at least 30 minutes with you – that’s personalized care.

We spend more time addressing your concerns whether its explaining how to use a glucose monitor for an accurate reading or answering questions about medications, insulin injections and meal planning. Our patients appreciate the amount of time and attention they receive at the clinic.

Our experts will customize a diabetes management program to fit your lifestyle

When you enroll in the Outpatient Diabetes Clinic program we take the time to diagnose, evaluate and provide a customized treatment plan that fits your lifestyle. Our nurse practitioners and certified diabetes educators work with you and your care team to simplify a complex condition, giving you the knowledge and confidence to control your diabetes.

We offer comprehensive resources and classes to help you understand how diabetes affects your body; how to overcome any challenges the condition may present; and reduce your risk for complications such as heart disease and stroke, high blood pressure, blindness and eye problems, kidney disease, dental disease, and more.

We provide counsel on:

  • Meal planning
  • Exercise counseling
  • Disease management
  • Managing stress
  • Insulin administration
  • Use of insulin pumps, blood glucose sensors and meters
  • Oral medications
  • Effects of illness on diabetes
  • Skin care

Diabetes Education Classes
We cover a wide array of topics, taught by nurse practitioners, registered nurses and dietitians. Topics include:

  • General overview of diabetes and pre-diabetic conditions
  • Introduction to insulin pumps and insulin pump starts
  • Assessment of diabetes control for insulin-pump users
  • Support groups
  • Medical nutrition therapy
  • Blood glucose self-monitoring
  • Exercise education

Ask your physician to send us a Patient Referral form for Diabetes Education so we can begin to schedule an appointment. Referrals should be faxed to 602.923.5814.

Learn how to take control of your diabetes. The program consists of three classes. Classes 1-3 should be taken sequentially.

Class 1

April 10, 10-11 a.m.; April 11, 2-3 p.m.
Overview of diabetes and meal planning.

Class 2 - April 17, 10-11 a.m.; April 18, 2-3p.m.
Monitoring treatment of high and low blood sugar, physical activity.

Class 3 - April 24, 10-11 a.m.; April 25, 2-3 p.m.
Medications, complications and coping with chronic disease.
